This encompasses all types of real time patient data stored in the cloud, so that everyone participating in or administrating care has a clearer view of the patients broader needs and eligible services.

We allow customizable HEDIS, RAF and MACRA measures as established by the  health systems. This allows providers, and health systems the ability to set goals and evaluate progress based on measures that are meaningful to them and their patients.

Our call center Case Managers ensure patients are getting medically necessary, quality care that is delivered efficiently and economically.

How it Works

The Alliance Health Information Exchange creates a network that allows all approved providers, organizations, and members of a patients care coordination team to access and update patient data that is approved and within their scope of practice. All of the data is updated in real-time with the EHR allowing better care coordination and clarity into patients needs across all caregivers.
